​Optage Home Care

Services in your home following a change in condition or post hospital stay are available to help you regain your strength and live your life to its fullest. Services may include but are not limited to:

Nursing assessments, pain and medication management, wound care, and more

Physical, occupational, and speech therapists for rehabilitation needs

Home health aides to assist with bathing, showering or other hands-on care

Homemaker/companion to help with laundry, light housekeeping, or other activities

Optage Senior Dining Choices

Enjoy affordable, home delivered meals with over 60 menu choices including diabetic, low-sodium and vegetarian options offering great taste and nutrition.

Optage Hospice

Experience support to face a life-limiting condition while living as fully and comfortably as possible in surroundings that are familiar: a private home, assisted living or memory care apartment, or skilled nursing care center—wherever you call home. Our team includes a nurse, hospice aid, social worker, chaplain and a medical doctor. Additional services to choose from include music, pet and massage therapies.

Genevive Primary Care

Through Genevive, we offer a care team led by a physician and nurse practitioner to deliver customized medical care in your home. By providing preventative care, Genevive improves overall health and helps reduce unnecessary trips to the ER or hospital. Genevive is a partnership between PHS and Allina Health.
