We offer skilled Nursing, Physical and Occupational therapy at home, when ordered by a physician. Our Home Health nursing and therapy staff can administer medications, change dressings, perform range of motion activities, and help create an individualized plan of care that allows patients to regain their independence and prior level of functioning, as well as have a successful transition from hospital or skilled nursing facility, to home and avoid further hospitalizations.
