Baptist Medical Group offers the full continuum of care for our patients not only in our clinics but also when you’ve been hospitalized and discharged home. We are always looking for ways to improve the care we provide and adding services to meet that goal.

Let us help you avoid worry and stress about things you may need after discharge. At Mississippi Baptist Medical Center, we want to continue to follow you even after a hospitalization. Our commitment to you is to provide continued follow-up through our Baptist At-Home Program until you are re-established with your regular primary care physician or established with a new one.

How does this benefit you?

You have been scheduled to be seen by a provider in the comfort of your home during the next few days. During the next 24 hours, you will receive a phone call from our dedicated team to answer any questions you may have.
Once the provider comes to see you, he/she will review the records from your recent hospital stay, review medications, follow up on any labs that need to be done, address any medical needs or concerns you may have, and provide education that is needed to ensure you are on track to a healthy recovery.
The provider will then communicate with your primary care provider to ensure he/she is updated and aware of your recent hospital stay. If you do not have an established Primary Care Provider, we can set up an appointment with a Baptist Medical Group provider. Copies of all medical records will be forwarded to your primary care provider if needed.
Don’t worry about this interfering with Home Health or any of the other services you may receive. We work together with Home Health and other services to keep you in the comfort of your home during your recovery.
