1915(i) Care Coordination


Prairie Harvest's 1915(i) Care Coordination program is an integral component of the Home and Community Based Services under Medicaid. Our Care Coordinator diligently works with individuals to create a person-centered plan of care, facilitating access to community resources and ensuring their overall needs are met. Key aspects of our service include ensuring continuity of care by collaborating with existing systems, connecting individuals to additional 1915(i) services, building informal support networks, providing equitable and culturally competent services, and empowering individuals by promoting choice, rights, and responsibilities. Our mission is to enhance the quality of life and promote independence for adults with serious mental illness.
