Rocky Mountain PACE (Program of All-Inclusive Care for the Elderly) is a specialized health plan designed for adults 55 and older who face multiple health challenges and want to remain in the comfort of their own home. Participants have access to primary medical care, medication management, physical therapy, specialty services, in-home care, social work assistance, and transportation to appointments and Day Centers, all tailored to their unique needs.
